“技术不发展，终归要走到尽头”，韩炳智称，经过十几年的发展，民营医院的院长也渐渐意识到这个问题，大家聚在一起经常讨论，医院如果要继续做大，未来肯定要在人才和技术上主动投入，至少要把原来那些不正规的东西去掉。
南方周末记者在宿迁走访发现，一些发展较好的民营医院，几乎都在大兴土木，扩建门诊楼、住院楼。人民医院2013年新投入使用了一座外科大楼，副院长张荣林称，投资了2.5个亿，全部靠的是医院自身十几年来的收入积累。
钟吾医院十几年发展下来，也一直在改善硬件。从原来租用的一片只有200张床位的平房区搬出，建设了自己的门诊大楼，500张床位的住院楼，目前一座新的住院楼仍在建设中。
除了硬件的改善，资本也在逐步换代升级。
2012年，钟吾医院和上市公司上海复星医药达成了合作，复星出资9900万，控股了钟吾医院55%的股权。
复星医药进入后，带来了更加先进的管理经验和更稳定的资金投入。仅仅一个财务报表，复星来的财务总监就提出来很多问题，管理越来越规范。
不过，即便是复星这样的医药上市公司控股后，在引进更优秀的人才和先进医疗技术上目前也还没有更多作为。“当初合作的时候，对方说我们有很强的医疗资源，可以帮助钟吾发展，但两年下来还没有明显的成效。”
韩炳智称，对方也牵过一两次线，但合作最终都没谈成。“都是上海南京很牛的公立医院，我们这个地方太小，这点回报人家也看不上，没有动力和你合作。”
韩炳智很期待国家正在推的医生多点执业政策，“如果医生真能流动起来，对我们或许是个好事”。
亲儿子和干儿子
就在此时，政府决定再次出手。
对于正在发展中的民营经济，是给予其足够的耐心，让其发展壮大，防止公立医院对民营医院发展的虹吸作用；还是及时弥补市场不足，政府再次出手，以公立医院的体制优势参与竞争？管理者能否在面对亲儿子和干儿子时，制定公平的竞争政策？
“这些问题，我们都想过”，程崇高对南方周末记者称，民营医疗机构是宿迁的招牌，政府肯定不会希望他们都死掉。因此，在第一人民医院招聘时，特别承诺不会从本地医疗机构中挖人才。
尽管有这样的承诺，还是让民营医院很担心，“现在医生都是自由流动的，人家就愿意去，政府难道不让去？”一位民营医院的院长对南方周末记者称。
事实上，确实也有部分当地民营医疗机构的医生选择跳槽到新建的第一人民医院，已经是宿迁最好医院的人民医院也有三四个人选择跳槽。
王士平是众多来自宿迁周边城市的应聘者之一。据其估计，目前已经招聘的近500个人中，大概有三分之一来自宿迁本地医院。有着近10年工作经验、刚刚深造后硕士毕业的普外科医生王士平很想进入一家大型公立医院磨练技术。
公立医院目前仍然是医生选择的主流，这从第一人民医院相比宿迁其他民营医院更容易的招聘就可以看出，“公立医院目前仍有巨大的优势，除非政府主动退出，否则民营医院根本没法和公立医院竞争”，王士平说。
出于对未来的种种担心，一位当地医院的负责人甚至给政府负责人打电话，请其对当初“不会再建公立医院”的承诺负责。
还有一些民营医院也伸手向政府要编制了，并称政府也在考虑，看能否给民营医院一些空编，实际不由财政负担。
既然由此带来一系列阻力和质疑，宿迁为什么不引入外来资本兴建这所三甲医院，而一定要自己来建？
程崇高称，宿迁的大门一直开着，事实上是没有人来投资，“宿迁决定建第一人民医院，是在2011年”，当时国家的医改还在强调医疗机构的公益性，社会办医政策尚不明朗，社会资本也没有像现在一样涌现。
但一位当地医疗系统人士对南方周末记者称，医院建设过程中，也有社会资本表达了想要参与的意思，卫生局也上报了，政府回复暂时先不考虑。
事实上，政府考虑出手，除了受困于宿迁优质医疗资源不足的困境，还有诸多其他现实的考虑。
比如，没有一个技术过硬的医疗机构存在，招商引资都有了瓶颈，程崇高称这是切肤之痛。再比如，遇到SARS、禽流感等公共卫生事件，民营医院动员起来总没有公立医院那么顺手。
即便是在现在，宿迁依旧是江苏经济最落后的地级市，排名一直垫底，境内至今没有铁路。举政府之力创办的这所公立医院，未来发展靠什么？
程崇高反复强调，现在给第一人民医院的一些特殊政策，都是为了在“亲儿子”还小时，扶上马，送一程。日后为了让其发展得更好，依旧要放到市场上竞争，肯定不会给特殊政策，“宿迁的门已经打开了，肯定不会再关回去了”。
既有公立医院、又有民营医院的宿迁医疗市场，是各取所长增加了宿迁医疗市场的丰富性，还是公立医院的政策优势减缓了宿迁民营医疗机构的发展？这些都有待时间检验。
尽管目前选择了公立医院，但王士平总感觉十年后的自己，技术修炼成功，还是会走向民营医院：政府财政压力加大，社保不堪重负，一定会打破公立医疗机构和民营医疗机构目前的格局。“现在的事业单位改制，就是破除公立医院体制优势的一个信号，到时民营医疗机构或许才能获得真正发展。”

Activities in this game are divided into several modules. First is the economy as a citizen in a country of your choice you must work to earn money, which you will get to spend for example, on food or purchase of weapons which are critical for your progress as a fighter. You will work in either private companies which are owned by players or government companies which are owned by the state. After progressing in the game you will finally get the opportunity to set up your own business and hire other players. If it prospers, we can even change it into a joint-stock company and enter the stock market and get even more money in this way.
In E-Sim, international wars are nothing out of the ordinary.
Become an influential politician.
The second module is a politics. Just like in real life politics in E-Sim are an extremely powerful tool that can be used for your own purposes. From time to time there are elections in the game in which you will not only vote, but also have the ability to run for the head of the party you're in. You can also apply for congress, where once elected you will be given the right to vote on laws proposed by your fellow congress members or your president and propose laws yourself. Voting on laws is important for your country as it can shape the lives of those around you. You can also try to become the head of a given party, and even take part in presidential elections and decide on the shape of the foreign policy of a given state (for example, who to declare war on). Career in politics is obviously not easy and in order to succeed in it, you have to have a good plan and compete for the votes of voters.
You can go bankrupt or become a rich man while playing the stock market.
The international war.
The last and probably the most important module is military. In E-Sim, countries are constantly fighting each other for control over territories which in return grant them access to more valuable raw materials. For this purpose, they form alliances, they fight international wars, but they also have to deal with, for example, uprisings in conquered countries or civil wars, which may explode on their territory. You can also take part in these clashes, although you are also given the opportunity to lead a life as a pacifist who focuses on other activities in the game (for example, running a successful newspaper or selling products).
